Commercial Truck Insurance Filings – Intrastate Transportation
Overview
If your business requires a federal or state filing, Barbee Jackson Insurance can help. In today’s highly regulated world it is important to work with an agency that has the experience and expertise not only to find you the right commercial trucking policy, but also to make sure your filings are correct with the Federal Motor Carrier Safety Administration (FMCSA) or state insurance office.
If you’re a business that operates within the state lines you may not be required to have a federal filing, but could be subject to state filings. The state filing will verify that you have the proper limits of liability insurance and/or cargo insurance.
State Filings – Exempt Commodities
If you haul an exempt commodity you are required to file an intrastate truck filing to show that you are maintaining state required limits of insurance.

States that require filings
California
MCP-65 – proof of insurance that you meet the required limits liability limits of California.
PL914 – proof of insurance for businesses that transport passengers for commercial balloons, rafting operations, and skiing businesses that transport passengers to ski resorts.
CA OL 207 – driving schools
TL 1000 – transporting passengers not-for-hire.
TL672 – required for household movers to provide proof of sufficient cargo insurance.
TL676 – required for household movers to provide proof of sufficient liability insurance.
Colorado
Form 12 and Form 14 are required for tow operations to prove that they meet the required limits of liability and on-hook coverage.
Connecticut
MC1641 – transporting passengers not-for-hire.
R1325C – proof of insurance for rented/leased vehicle for commercial purposes.
GU1327C – notifies the state of changes in limits to your liability insurance.
Montana
HQMV-5 – required for tow operations to provide proof of liability insurance.
Ohio
OS-32 – required for oversized vehicles/trailers to provide proof of sufficient liability insurance.
Oklahoma
WA Form – requires tow operations to provide proof of liability insurance.
Rhode Island
GU1338C – required to verify proof of liability insurance or financial responsibility that is vehicle specific and must be issued for each vehicle separately.
Texas
Form T – required for tow operations to provide proof of liability insurance.
Intrastate Commercial Auto Insurance Filings:
Form E: Verifies commercial auto insurance meets state limits.
Form F: Endorsement to the commercial auto policy that adjusts coverage to meet financial guidelines in your state.
Form H: Verifies you have sufficient cargo liability insurance.
Form K: Cancels previous state insurance filing.
